Short-Term Transitional Care
Think of transitional care as a bridge to becoming well. At NERNC we focus on helping you make the transition from acute hospitalization to home. In fact, we provide an atmosphere much like home, where you can practice taking your medications or using a walker, for example, if necessary. Our goal is to have you leave our care stronger, healthier, and more self-sufficient.

Services include short-term rehabilitation, respite care, and specialized wound care. Patients or residents who are admitted to NERNC must be medically stable. Common diagnoses of patients include mild strokes, joint replacements, arthritis, and patients who require intravenous antibiotics.
