If you will soon be admitted to the North End Rehabilitation
and Nursing Center (NERNC) this section is for your benefit. By now, you
may have met the NERNC liaison representative who has answered your questions
and told you what to expect.
Because we understand how confusing thetransition from
the hospital to a rehabilitation center or skilled nursing facility can
be, we have gathered these facts to provide an overview of NERNC’s
services. We hope that this will ease your transition and help you to
settle into your new surroundings.

- Clothing. You will be actively involved in therapy. Bring loose, comfortable
clothes and shoes with rubber, non-skid soles. In addition, bring rubber
shower sandals. It is advisable to label your clothing. Laundry services
are available.
- Valuables. DO bring enough money to pay for newspapers or hairdresser/barber
services. Please leave expensive jewelry, or large sums of cash, at
home. You will also be provided a locked drawer for valuables.
- Toiletries. NERNC provides some basic toiletries, such as a toothbrush,
toothpaste, powders, and lotions, but feel free to bring
your personal favorites.
- Medical Equipment. Including CPAP, BIPAP, reading glasses, hearing
aids, and customized assistive devices etc…
- Items or equipment for relaxation activities,such as radio/CD player
with headset, books, magazines, knitting needles, or playing cards.
- A Health Care Proxy, if you have one.
- A positive attitude for your rehabilitation stay.

- If you are a subacute patient, you will have a telephone and a television
in your room (charges apply).
- 3rd and 4th floor residents have telephone and cable access, though
TVs are not provided.
- You can receive a daily newspaper (charges apply).

- If you are being transferred from another hospital, you will be transported
to NERNC by ambulance or chair car.
- During your stay at NERNC, medically necessary transportation to
medical appointments may be provided.

- Your assigned care team will include a physician, a staff nurse practitioner,
a case manager, physical and occupational therapists, speech/language
pathologists, social services, dietician, and activities staff.
- Your care will be coordinated by a primary nurse practitioner with
oversight from your physician (who you may or may not see daily depending
on your condition). Your physician or nurse practitioner will meet with
you within the first 24 hours of your stay. You will meet your other
team members either that day or the next.
- You will receive information that explains what you and your family
should expect during your stay. This information will familiarize you
with NERNC’s patient and visitor services, and will help you and
your family to understand your rehabilitation process.

- Your recommended length of stay will be determined by your care team,
based on your progress. You and your family will be included
in the decision-making process. Typical stays are less than two weeks.
Our goal is for you to be as safe as possible to return to your previous
living arrangement.
- Your treatment team determines the amount of therapy you will receive.
Therapists are on site 7 days per week but your therapy visit frequency
may vary. We encourage family members to take part in your therapy,
as they can provide support and motivation and can learn skills to prepare
you for your transition home. Family/care team conferences will be scheduled
regularly to discuss or reassess your
treatment program.
- You can spend time with your family and friends during visiting hours.

Your family members can tour the NERNC before your transfer. To schedule
